Welcome! By scheduling your first appointment, you are taking a major step forward in your health journey.

Dr. Althaus’ dedication to finding the root cause of patient ailments is reflected in her appointment times: an initial appointment is scheduled for 60 minutes to allow ample time to discuss & address each health concern.

At this time, Revive Health is conducted solely via telemedicine (video) visits.

The Process

All potential patients are required to fill out a New Patient Application Form to verify that they are eligible to receive care as part of Revive Health Foundation. Once this form is complete, you can submit a request for your first office visit with Dr. Tiffany Althaus.
